To speed up the initial consultation, you can download and complete the Health History & Consent Form. You may also have easier access to information if you complete this at home. There are two versions of the form – one for Massage Therapy which can be downloaded by clicking here. The second one is for Bowen Technique and that can be downloaded by clicking here. If there are any parts you are unsure about, just leave them blank and we can discuss when you come into the clinic.
